    Manage Your Repairs

    We believe that diligence in providing our customers with the most accurate, detailed information will result in lower repair costs. Life Systems offers the following services to assist you in making educated decisions regarding the care of your equipment and budgeting for future needs.

    PMI's (Preventative Maintenance Inspections) - Our repair service representatives take a proactive role in maintaining your equipment by providing educational in-services and PMI's. You will be furnished with a copy of the PMI upon completion and can make informed decisions about areas of potential concern. PMI's have proven to limit the frequency of major repairs by identifying minor problems before they develop into major ones.

    Service Reports - Using software specifically designed to serve you, Life Systems provides detailed information about the condition of your equipment. Upon completion of each repair, you will receive a service report that accurately describes the work performed and brings to light any potential problems.

    We also provide quarterly and annual reports which summarize the repairs performed on your equipment. This timely information assists you in making decisions about the management of your equipment as well as budgeting for future expenditures.

    The Life Systems Repair Service Center is a state-of-the-art repair facility located in the St. Louis metro area.
    All minor and major endoscope repairs are performed on site.

    To Send a Repair

    ALL equipment MUST be decontaminated prior to sending in for repair.

    To arrange for the repair of your endoscopic equipment, request a prepaid shipping label or schedule a pick-up, please contact the service center. Your customer service representative will review the procedure for sending in your equipment. If you would like to request a loaner, please go to the Loaner Page.

    To expedite the repair of your equipment, please fill in the information fields on the Life Systems Equipment Repair Order Form.
    Print it out, sign approval area and send in with the equipment needing repair.

    To simply do a Repair/Exchange, please fill out the following form: Equipment Repair Exchange Form.
    Print it out, sign approval area and send in with the equipment needing repair. An exchange piece of equipment will be sent out promptly.

    Required information for both forms:

    • Account information including contact name and telephone number

    • Purchase order number

    • Brief description of the problem

    • Model number and serial number of the equipment

    Please ship your equipment securely in the proper packaging with the appropriate paperwork.
    Make sure that the equipment has been cleaned, disinfected and/or sterilized.

    When Your Equipment Arrives at Life Systems

    Upon arrival to Life Systems, your equipment is inspected and evaluated. Before any work is performed, you will be contacted to review the repairs which are necessary to return your equipment to manufacturer's specifications. Also, any other problems identified during the initial evaluation will be brought to your attention.

    The repair estimate may be communicated via a telephone call, fax or e-mail. Upon receipt of your signed approval, repairs will commence.
